Before You Book
- Self-pay model. Insurance is not accepted, though HSA/FSA cards are. Medication costs are billed separately through partner pharmacies.
- Choose the cadence carefully. Monthly Membership gives four draws and four consults per year. Core Annual is cheaper, but lighter: two draws and one consult.
- Refunds are limited after care starts. The patient agreement says refunds are not owed once care has begun or a membership has been paid in full.
- Advisors are not the treating team. High-profile founders and medical advisors shape the program; member visits are handled by Lifeforce clinicians and coaches.

Lifeforce has several entry points. Monthly Membership is $199/mo for the first month, then $149/mo, with four blood draws and four clinician consults per year. Core Annual is $599/yr with two draws and one consult. Annual Full is listed at $1,449/yr for the higher-cadence membership paid upfront. The TRT program starts at $49/mo for the first month, then $99/mo; Weight Loss, Menopause, Cardiac, and Metabolic programs start at $149/mo. The One-Time Diagnostic is $599/mo list or $349 through the Ethos promotion. Medications and advanced panels are billed separately, so real monthly spend depends on the plan. Where is Lifeforce available?
All 50 US states. Medical services are delivered through state-specific professional corporations: Lifeforce Medical NJ, P.C. (New Jersey), Lifeforce Medical KS, P.A. (Kansas), Van Maele Medical, P.C. (California), and Lifeforce Medical, P.A. for all other states. International members are not supported.
Does Lifeforce accept insurance?
No. Lifeforce is self-pay only. HSA and FSA cards are accepted for both memberships and the One-Time Diagnostic.
Does Lifeforce prescribe testosterone and other controlled substances?
Yes. Lifeforce offers Schedule III testosterone through its TRT program, including injection, cream, and oral Kyzatrex options. GLP-1s, peptides, HRT, metformin, statins, and related prescriptions can also be prescribed when clinically appropriate. State-specific prescribing limits are not fully published, so confirm eligibility during intake if a specific medication is central to your plan.
How often does retesting happen?
Monthly Membership retests quarterly, with four blood draws and four clinician consults per year. Core Annual retests twice yearly and includes one clinician consult.
Is there a commitment or cancellation policy?
